It might help to have a trusted friend or family member help your patient deal with insurance issues.
Insurance company websites will typically have a downloadable authorization form for your patient to sign to authorize someone to help.
Your patient can ask a customer service rep to provide an authorization form or download a document on their website called:
- Designation of Representative Form;
- Authorization Form;
- Authorized Representative Designation Form; or
- Something similar
